Applying to Kindergarten - Grade 8
This checklist will guide you through the application process.
- Attend an Open House / Tour SMS
- Complete an Application Form and submit non-refundable $50 fee/per child.
- Arrange for completion of Teacher Observation Form (Kindergarten only) or two Teacher Recommendation Forms (Grade 1 - Grade 8)
- Complete an Authorization for Release of Records (Grade 1-Grade 8) to obtain transcripts
- Arrange for student to visit SMS and have Screening
If your child is accepted, you will need to:
- Complete Enrollment Form and submit non-refundable $300 Enrollment Fee/child
- Submit Kindergarten Commitment Fee of $500, which will be applied to Grade 1 Tuition
- Provide copy of birth certificate and baptismal certificate
- Submit completed State of Connecticut Health Assessment Record
- Complete Smart Tuition application and return with Enrollment Form.
