Current SMS Families Enrollment for 2013-2014
We will be happy to welcome current SMS families to the enrollment process for the 2013-2014 School Year beginning January 7, 2013. The process will be as follows:
Returning Preschool Students:
- Complete Enrollment Form and submit non-refundable $150 Enrollment Fee/child by February 4, 2013
- Submit newly completed State of Connecticut Early Childhood Health Assessment Record
Returning Kindergarten Students:
- Complete Enrollment Form and submit non-refundable $150 Enrollment Fee/child
- Submit Kindergarten Commitment Fee of $500, which will be applied to Grade 1 Tuition
- Arrange for completion of Teacher Observation Form
- Submit newly completed State of Connecticut Health Assessment Record
- Provide copy of Baptismal Certificate
Returning Grade 1 - Grade 8 Students:
- Complete Enrollment Form and submit non-refundable $150 Enrollment Fee/child
Siblings of Current SMS Students
Siblings of Current SMS Students have priority in the Admissions Process.
Sibling Applying to Preschool:
- Complete Enrollment Form and submit non-refundable $150 Enrollment Fee/child
- Provide copy of Birth Certificate
- Submit completed State of Connecticut Early Childhood Health Assessment Record
Sibling Applying to Kindergarten - Grade 8:
- Attend an Open House / Tour SMS (optional)
- Complete an Application Form and submit non-refundable $50 fee/per child.
- Arrange for completion of Teacher Observation Form (Kindergarten only) or two Teacher Recommendation Forms (Grade 1 - Grade 8)
- Complete an Authorization for Release of Records (Grade 1-Grade 8) to obtain transcripts
- Arrange for student to visit SMS and have Screening
If your child is accepted, you will need to:
- Complete Enrollment Form and submit non-refundable $150 Enrollment Fee/child
- Submit Kindergarten Commitment Fee of $500, which will be applied to Grade 1 Tuition
- Provide copy of birth certificate and baptismal certificate
- Submit completed State of Connecticut Health Assessment Record
